The Advance Practice Provider (APP) serves as an additional patient care provider, augmenting and extending patient services provided by the health care team.
The APP will exercise his/her patient care and medical decision making autonomy within established clinical parameters regulated by the New York Board of Health Professions.
The APP will provide a broad range of diagnostic, therapeutic, and preventive services including history and physical exams, diagnosis and evaluation of specific illness and/or injuries, and effective treatment of diseases or injuries, both chronic and acute, and preventive and health maintenance services.
Qualifications NYS licensed Nurse Practitioner or Physician Assistant.
One year of experience in primary care preferred.

Saratoga Hospital SHMG - Primary Care Milton Schedule:
Full-Time, 8-5.
Estimated Salary: $20 to $28 per hour based on qualifications.
